Job summary

The Landings of Oak Island is seeking a dedicated Resident Care Coordinator to join our team and ensure the health and well-being of residents through coordinated care plans. You will manage ISPs in collaboration with clinical staff, residents, and families to deliver person-centered care.

You will work with the Executive Director, healthcare providers, and care staff to promote a safe, nurturing environment and maintain compliance with state regulations and industry best practices.

Qualifications

• Associate’s or Bachelor’s degree in Nursing, Healthcare Administration, or related field preferred.
• Active LPN or RN license preferred (depending on state requirements).
• Minimum of 2 years of experience in a senior living, assisted living, memory care, or healthcare setting.
• Previous supervisory or leadership experience in a healthcare environment is highly desirable.
• Strong leadership, communication, and interpersonal skills.
• Ability to manage multiple priorities in a fast-paced environment.
• Knowledge of state regulations and compliance standards for assisted living and memory care.
• Proficiency in electronic health records (EHR) systems and basic computer skills.

Responsibilities

• Care Plan Development & Management: Assess residents' needs and develop ISPs with clinical staff, residents, and families.
• Staff Supervision & Training: Lead, train, and guide caregiving staff; assist with scheduling to ensure adequate coverage.
• Resident & Family Engagement: Be primary contact for care-related concerns; coordinate family meetings and care conferences.
• Regulatory Compliance & Quality Assurance: Ensure compliance with state regs and company policies; participate in audits.
• Collaboration: Work with nursing, physicians, therapists, and leadership to coordinate care; support operations.
