Qualifications

• 2+ years in senior living, assisted living, memory care, or healthcare setting.
• Active LPN or RN license preferred (state requirements vary).
• Strong leadership, communication, and interpersonal skills.
• Proficiency with electronic health records (EHR) systems.

Responsibilities

• Develop and manage individualized care plans in collaboration with clinical staff, residents, and families.
• Provide leadership, training, and guidance to caregiving staff; assist with staffing to ensure quality care.
• Serve as primary contact for residents and families regarding care-related concerns; facilitate family meetings and care conferences.
