Job summary

RHA Health Services is seeking a Resident Care Coordinator to coordinate medical, psychiatric, and dental appointments and communications with care teams. This role guides residents toward independent living and supports development of individualized plans.

The position requires travel between sites and at least an associate degree with 4 years of experience, or a bachelor’s with 2 years, or a master’s with 1 year. Regular schedule Mon–Fri 8am–4pm.

Qualifications

• Master’s degree or higher with 1 year of relevant experience, or Bachelor’s with 2 years, or Master’s with 1 year.
• Degree in related field required for direct support roles.
• Experience serving individuals with intellectual/developmental disabilities or autism is preferred.

Responsibilities

• Coordinate and schedule medical, psychiatric, and dental/vision appointments with providers.
• Communicate with physicians, nurses, and behavioral health staff regarding residents’ medical needs.
• Record and track health care activities and maintain required documentation.
• Perform Direct Support Services when needed.
• Travel between sites to support residents and programs.
