Job summary

Webster Manor Rehabilitation & Health Care Center, part of Athena Health Care Systems, is hiring a full-time Resident Care Coordinator Assistant (RCCA) in Webster, MA.

The RCCA will support the Resident Care Coordinator with MDS-related duties, ADL tracking, and care plan coordination. A CNA license and MDS familiarity are required or preferred, with English proficiency essential for daily interactions with residents and families.

Qualifications

• Must read, write, speak and understand English.

Responsibilities

• Assist RCC in day-to-day MDS processes.
• Enter demographic data into MDS software.
• Complete discharge and re-entry MDS forms.
• Schedule care plan meetings and invite families.
• Coordinate teleconferences with families when requested.
• Monitor Medicare certifications and recertifications.
• Prepare and file MDS and care plans.
