Webster Manor Rehabilitation & Health Care Center in Webster, MA is hiring a full-time Resident Care Coordinator Assistant to support the RCC in day-to-day MDS-related activities and resident assessments.

The role requires CNA license, knowledge of medical terminology, and strong organizational skills; duties include education for CNAs, MDS entry, meeting coordination, and interfacing with families and care teams. Compensation is $22 per hour with weekly pay and benefits.

Qualifications

• Must read, write, speak, and understand English.
• Knowledge of medical terminology, filing, typing, computer, and organizational skills.
• Active CNA license and experience is required.
• A high school diploma or its equivalent.

Responsibilities

• Assists RCC with day-to-day MDS-related activities and resident assessments.
• Enter demographic information into MDS software and complete discharge/re-entry forms.
• Schedule care plan meetings and coordinate with family members.
• Communicate with interdisciplinary team and manage MDS-related documentation.
