Job summary

Civita Care Center in Waterford, CT is seeking a full-time Resident Care Coordinator Assistant (RCCA) to support the Resident Care Coordinator in the daily MDS process. The role offers weekly pay and union benefits as part of Civita Care Systems’ commitment to staff development and quality of care.

Responsibilities include data entry into MDS software, coordinating care plan meetings, and ensuring accurate ADL coding with CNAs while supporting family communications.

Qualifications

• Must read, write, speak, and understand English.
• Ability to make independent decisions when warranted.
• Must deal tactfully with residents, families, and staff.
• Willingness to adopt new methods and practices.

Responsibilities

• Assists RCC with day-to-day MDS activities.
• Enters demographic information into MDS software system.
• Schedules care plan meetings and coordinates with family members.
• Educates CNAs on ADL coding and tracks progress.
• Monitors Medicare certifications and recertifications.
