Job summary

Brigham House is seeking a Resident Care Director Assistant to evaluate residents' health functioning and design individualized care plans. This role ensures compliance with applicable federal and state regulations and supports the wellness program across the community.

The position involves supervising medication management, coordinating health-related services, and building strong relations with residents, families, and physicians while ensuring proper scheduling and staff development.

Qualifications

• Must be licensed in appropriate State as LPN, RN Preferred.
• Able to apply common sense understanding to carry out detailed but uninvolved written or oral instructions.
• Able to work various schedules and shifts as needed.
• Must have a solid understanding of the Wellness department, Policies and Procedures, regulation requirements.

Responsibilities

• Evaluate and assess the health functioning of residents and design and revise care plans.
• Assist in supervising the medication management program and assist residents with self-administration of medications.
• Coordinate health-related services for residents.
• Develop service plans per state regulations and organizational policies.
• Supervise health-related activities and staff, including scheduling, training, and evaluations.
• Complete RCA and Wellness nurse schedules and report changes to the Executive Director.
