Job summary

You will work as part of the neighborhood care team, following care plans, documenting changes, ensuring safety, and promoting psychosocial well-being through relationships and activities.

Qualifications

• High School diploma or GED required.
• Current CPR certification preferred.
• One year of Personal Care Worker experience preferred.

Responsibilities

• Provide personal care in a calm, professional and clinically proficient manner.
• Assist with dining experiences, including serving, cueing and feeding.
• Observe residents, report changes to supervisor promptly.
• Assist with activities of daily living and mobility as needed.
• Maintain resident confidentiality and safety; follow care plans.
• Support neighborhood programming and resident activity participation.
