Job summary

Great Lakes Management in Maplewood is seeking a Resident Assistant to provide hands-on care and support to residents according to their service agreements. You will assist with daily living activities, encourage independence, and offer emotional support.

Responsibilities include assisting with dining, participating in activities, documenting changes, and clear communication with families and the case manager as needed. Prior long-term care or CNA certification is preferred.

Qualifications

• Six months of experience in long term care or assisted living (Preferred).
• Registered as a Nursing Assistant, Home Health Aide or Certified Nursing Assistant (Preferred).
• Ability to make responsible choices and act in the resident’s best interests.

• Assist residents with ADLs as indicated on the service agreement.
• Help residents reach and maintain the highest level of independence.
• Provide residents with physical and emotional support.
• Assist with treatments as delegated by the Registered Nurse per the service agreement.
• Observe clients and report changes to the case manager.
• Interact with residents to provide reassurance, engagement, and redirection.
• Complete all required documentation daily, service schedule, flow sheet, communication book, etc.

Job Objective

The Resident Assistant is responsible for providing hands‑on care, both physical and emotional, as outlined in each resident’s service agreement. Responsible for effectively communicating with residents, families, and notifying the case manager of any changes in condition or concerns. The Resident Assistant assists in the dining room and participating in activities and outings.

Job Responsibilities

• Assist residents with ADL’s (Activities of Daily Living) as indicated on the individualized service agreement
• Help each resident to reach and maintain the highest level of performance and independence.
• Provide residents with both physical and emotional support.
• Assist with treatments as delegated by the Registered Nurse per the resident service agreement.
• The Resident Assistant observes clients and reports changes in physical and emotional condition such as change in attitude, loss of appetite, weight, etc. to the case manager.
• Interact with residents to provide reassurance, engagement, and redirection as appropriate.
• Complete all required documentation daily, service schedule, flow sheet, communication book, etc.
