Job summary

The Fountains at Hosanna in Lakeville, MN is seeking a compassionate caregiver to provide personal care and health-related services under the direction of a Registered Nurse. You will assist with daily living activities, monitor residents, and support meal service and social activities in a warm, friendly environment.

Responsibilities include bathing, grooming, dressing, transfers, medication assistance under RN supervision, vitals, and documenting observations.

Qualifications

• 1 year caregiver experience.
• Ability to use technology in the form of computers, handheld devices, cell phones, and fax machines.
• If under 18 must be MN NA registry and the site must manage restrictions that prevent medications or lift operation.

Responsibilities

• Assist residents with personal cares: bathing, grooming, dressing, toileting, feeding.
• Assist with transfers and use of mechanical lift (1-2 people).
• Administer medications under RN direction and record vitals as directed.
• Perform home services including laundry and meal tray delivery.
• Support meal service in dining room and assist with activities as requested.
• Observe residents and report changes to staff and respond to emergencies.

Job Overview

The Fountains at Hosanna is conveniently located in Lakeville adjacent to Hosanna Church. We are less than a mile from Highway I-35 and near County 77. The Fountains offers seniors 92 Independent Living, Assisted Living, and Memory Care apartments. The Fountains and their exceptional staff are known for creating a warm, friendly, fun community.

This is a casual role!

This position is responsible for providing personal care and designated health-related services to enhance and/or maintain the residents' physical and emotional wellbeing.

Responsibilities

• Assists residents with personal cares including but not limited to:
• Bathing/showering
• Foot and fingernail care
• Shaving
• Dressing
• Assistance with use of hearing aids and/or glasses
• Hair care
• Feeding
• Toileting
• Escorts
• 1-2 person transfers and use of mechanical lift
• Provides health-related services as directed: Assists with treatments per individualized care plan under the direction of the Registered Nurse. (includes blood glucose monitoring)
• Administers medication or assists with this under the direction of the Registered Nurse (includes insulin injections). Takes and records temperature, blood pressure, pulse and respiration under the direction of the Registered Nurse
• Performs specified home services:
• Performs laundry service per schedule.
• Follows cleaning schedule for the shift worked.
• Delivers meal trays.
• Assists with meal service in dining room.
• Assists with preparing for and/or facilitating resident social and recreational activities as assigned and as requested.
• Observes tenants/residents and reports status to appropriate staff.
• Notifies appropriate staff of changes in physical and/or emotional conditions such as change in attitude or mood, loss of appetite, change in weight.
• Serves as a mandated reporter and reports any concerns regarding potential mistreatment
• Offers pertinent observations and input.
• Responds appropriately to resident and building emergencies.

• Ability to use technology in the form of computers, handheld devices, cell phones, and fax machines
• If under 18 must be on the MN NA registry and site must be able to manage restrictions that they are not able to pass medications or operate a mechanical lift independently.

Preferred Qualifications

• 3 years experience
