Job summary

Ebenezer, York Gardens, Minnesota, is seeking a compassionate caregiver to provide personal care and health-related services to residents in Assisted Living, Memory Care, and Enhanced Care settings. The role requires caring for residents during night shifts (Fri-Sat) and performing tasks under RN direction, including medication support and vital signs.

Responsibilities include bathing, dressing, feeding, mobility assistance, and reporting any changes in condition.

Qualifications

• Demonstrated ability to provide personal care tasks (bathing, dressing, grooming)
• Assist with medications and document under RN guidance (insulin injections, vitals)
• Observe residents and promptly report changes to staff

Responsibilities

• Assist residents with bathing, dressing, feeding, toileting, and mobility
• Support with meal service and dining room assistance
• Provide and document health-related services per care plan under RN supervision
• Assist with laundry, cleaning, and adherence to shift schedule
• Escort residents and assist with transfers using lifts when required
• Observe and report changes in residents’ condition and respond to emergencies as needed

Job Overview

York Gardens - We have 76 Assisted Living, Memory Care and Enhanced Care apartments in a warm, friendly, convenient community.

Week 1: Fri - 10:30pm -6:30am

Week 2: Sat & Sun - 10:30pm -6:30am

This position is responsible for providing personal care and designated health-related services to enhance and/or maintain the residents' physical and emotional wellbeing.

Responsibilities

• Assists residents with personal cares including but not limited to:
• Bathing/showering
• Foot and fingernail care
• Shaving
• Dressing
• Assistance with use of hearing aids and/or glasses
• Hair care
• Feeding
• Toileting
• Escorts
• 1-2 person transfers and use of mechanical lift
• Provides health-related services as directed: Assists with treatments per individualized care plan under the direction of the Registered Nurse. (includes blood glucose monitoring)
• Administers medication or assists with this under the direction of the Registered Nurse (includes insulin injections). Takes and records temperature, blood pressure, pulse and respiration under the direction of the Registered Nurse
• Performs specified home services:
• Performs laundry service per schedule.
• Follows cleaning schedule for the shift worked.
• Delivers meal trays.
• Assists with meal service in dining room.
• Assists with preparing for and/or facilitating resident social and recreational activities as assigned and as requested.
• Observes tenants/residents and reports status to appropriate staff.
• Notifies appropriate staff of changes in physical and/or emotional conditions such as change in attitude or mood, loss of appetite, change in weight.
• Serves as a mandated reporter and reports any concerns regarding potential mistreatment
• Offers pertinent observations and input.
• Responds appropriately to resident and building emergencies.

Required Qualifications

• less than 1 year
• Ability to use technology in the form of computers, handheld devices, cell phones, and fax machines
• If under 18 must be on the MN NA registry and site must be able to manage restrictions that they are not able to pass medications or operate a mechanical lift independently.
