Job summary

Plymouth Rehabilitation & Health Care Center is hiring a compassionate social worker to join our social services team. Shift is 8:30a-4:30p, Weekly pay ranging from $34 to $38, depending on license and experience.

As a Social Worker, your role is to plan, organize, develop, and direct all required social services to meet the needs of residents to ensure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.

Qualifications

• Must possess a social work license in MA (LSW, LICSW, LSWA, LCSW) or be in process.
• Must possess a bachelor's in social work from an approved school.

Responsibilities

• Coordinate and monitor resident care conferences.
• Work with residents, responsible parties, and families toward adjustment to placement and crisis.
• Protect residents’ health information and adhere to Privacy Practices.
• Complete comprehensive documentation in a timely manner.
• Advocate coordinate personal needs and utilize community resources.
• Support staff regarding resident-related issues.
• Perform other duties as assigned by supervisor.
