Beth Israel Deaconess Hospital Plymouth is seeking a Per Diem Social Worker to provide psychosocial assessments and brief therapeutic interventions. You will collaborate with the care team to address barriers to well‑being and manage length of stay.

Qualifications

• MSW degree required.
• LICSW licensure required.
• Minimum four years of social work experience in healthcare, psychiatric and substance use settings.

Complete the Case Management Initial Assessments for patients assigned to Social Work upon admission. Provide consultation to members of the Care Team on bio-psychosocial, environmental and cultural factors. Provide brief therapeutic intervention with patient and families around the issues of diagnosis, adjustment, grief or loss. Utilize appropriate case management interventions to address current and/or future barriers to care and well-being; facilitate referral to support services to promote well-being and manage length of stay. Upon patient admission, initiate the process of developing a discharge plan with patient, family and medical interdisciplinary team which will result in a safe disposition for the patient. Facilitate family and treatment team meetings early in the admission, as clinically indicated. Participate in supervision and team meetings. Frequent keyboard and telephone use. Constant concentration and problem solving.Pay Range:$28.90 – $51.34This position is covered by a collective bargaining agreement. The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. The pay range for this position is established and governed by the terms of the applicable collective bargaining agreement.
