Job summary

Northwood Rehabilitation & Health Care Center in Lowell is seeking a Social Worker to plan, coordinate, and direct social services for residents, ensuring their medical, emotional, and social needs are met on an individualized basis.

This 32-hour-per-week role offers a licensing path in Massachusetts and involves coordinating care conferences, advocating for residents, and utilizing community resources to support placement and recovery.

Qualifications

• Must possess a social work license in Massachusetts (LSW, LICSW, LSWA, and/or LCSW).
• A minimum of one (1) year social work experience in a hospital, skilled facility, or health-related field preferred.
• Must possess a bachelor's degree in social work from an approved school of social work.

Responsibilities

• Coordinate and monitor resident care conferences.
• Work with residents, responsible parties, and families toward adjustment to placement and crisis scenarios.
• Comply with privacy policies and protect residents' health information.
• Complete comprehensive documentation in a timely manner.
• Advocate and coordinate residents' personal needs.
• Utilize community resources appropriately.
• Support staff on resident-related issues.

Job description

We are hiring a Social Worker to join our dynamic care team at Northwood Rehabilitation & Health Care Center in Lowell!

32 hours a week on 8:30a-4:30p. Weekly pay ranges from $34 to $38 an hour, depending on license and experience.

As a Social Worker, your role is to plan, organize, develop, and direct all required social services to meet the needs of residents to ensure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.

Experience & Education:

• Must possess a social work license in the state of Massachusetts (LSW, LICSW, LSWA, and/or LCSW).
• A minimum of one (1) year social work in a hospital, skilled facility, or other health-related field preferred.
• Must possess, as a minimum, a bachelor's in social work from an approved school of social work.

Duties & Responsibilities:

• Coordinate and monitor resident care conferences.
• Work with individual residents, responsible parties, and families towards adjustment to placement and to crisis (i.e., illness, disability, and death).
• Complies with facility Privacy Practices policies and procedures and protects residents’ individual health information.
• Complete comprehensive documentation in a timely manner.
• In the role of advocate coordinate personal needs.
• Utilize community resources appropriately.
• Support staff in relation to resident-related issues.
• Other duties deemed appropriate and assigned by your supervisor.
