Job summary

Complete Care in Wisconsin is seeking a Social Services Director / Social Worker to advocate for residents, assess psychosocial needs, and coordinate care with a multidisciplinary team. You will document and monitor care plans and ensure rights and dignity are upheld.

Qualifications

• Bachelor’s in Social Work (BSW) or MSW required.
• Active Wisconsin license (CSW/APSW/ISW/LCSW) or equivalent.
• Minimum 1 year supervised social work in healthcare; LTC/SNF preferred.

Responsibilities

• Assess residents’ psychosocial, mental and emotional needs through visits and observation.
• Complete social histories and assessments upon admission and regularly thereafter.
• Integrate findings into interdisciplinary care planning (MDS) to address needs.
• Advocate for residents’ rights, dignity and self-determination.
• Prevent and address abuse, neglect and exploitation as required by law.
• Coordinate discharge planning and post‑discharge resources.
• Provide emotional support to residents and families.
• Other duties as assigned by the Administrator.
