Qualifications

• Bachelor’s Degree in Social Work (BSW) is required.
• Active, unencumbered LSW or CSW license in Wisconsin.
• 2+ years in LTC or SNF preferred.
• High proficiency in EHR and solid grasp of MDS 3.0 requirements.
• Strong emotional intelligence and ability to mediate complex family dynamics.

Responsibilities

• Lead psychosocial assessment and holistic care planning.
• Direct the Social Services portion of the MDS process.
• Navigate complex discharges with community resources to ensure smooth transitions.
• Educate staff and families on resident rights and self-determination.
• Provide crisis intervention and counseling to residents and families.
• Ensure all documentation meets state and federal standards, audit-ready.
• Develop community networks with Madison-area hospitals, VAs, and home health agencies.
