CommuniCare Health's Sellersburg Healthcare Center in Sellersburg, IN is seeking a Social Services Designee to enhance residents' quality of life. You will collaborate with Social Services, Therapy and clinical teams to plan discharge and advocate for residents.

The role emphasizes building rapport with residents and families, reporting grievances, maintaining confidentiality, and ensuring timely communication with the RN Charge Nurse.

Qualifications

• High school diploma or GED required.
• Previous social services experience in a long term care environment.

Responsibilities

• Work with Social Services, Therapy and clinical teams to establish discharge planning for residents.
• Build relationships with the team and residents to support residents' quality of life.
• Maintain good rapport with direct caregivers to ensure participation in programs/services for residents' psychosocial well‑being.
• Report grievances and complaints with oral/written notes to the Director of Social Services.
• Serve as the resident’s advocate, coordinating with direct care staff to meet needs at all times.
• Monitor changes in residents' condition and report promptly to the RN Charge Nurse.
• Provide information to the Director of Social Services to help solve resident needs.
• Suggest improvements to services and activities to increase resident engagement.
• Collaborate with the activity department to involve family members and guests in activities.
• Maintain confidentiality of all resident/family information.
• Perform general social services duties as required.
