Job summary

Blue River Services, Inc. is seeking a Case Manager/QIDP to coordinate person-centered services for residents in residential supports. The role develops, monitors, and updates service plans, advocates for residents, and collaborates with staff, families, and agencies to ensure compliant services.

The candidate will lead assessments, maintain documentation, and support residents with benefits and notifications. A bachelor’s in human services and QIDP eligibility are required.

Qualifications

  • Bachelor’s degree in a human services field is required.
  • Must meet qualifications to serve as a Qualified Intellectual Disabilities Professional (QIDP).
  • Minimum one year of experience with individuals with developmental disabilities.
  • CPR, First Aid, CPI certification required; valid driver’s license and reliable transportation.

Responsibilities

  • Lead annual assessments and interdisciplinary team meetings for each resident’s plan.
  • Develop, update, distribute, and maintain Individual Service Plans and documents.
  • Monitor progress, complete quarterly evaluations, revise plans as goals change.
  • Coordinate with Home Managers to ensure plans are implemented.
  • Advocate for residents’ rights, preferences, and community participation.
  • Collaborate with residential, vocational, therapy, families, guardians, and agencies.
  • Maintain resident records, confidentiality, and state-agency reports.
  • Assist with corrective action plans and compliance requirements.
  • Provide on-call crisis intervention and resident support as needed.
  • Assist residents with benefit applications and required notifications.
