Job summary

You will develop, monitor, and update service plans, advocate for residents, and collaborate with staff, families, and state agencies to ensure quality, compliant services. The role requires a Bachelor’s degree in a human services field, QIDP qualifications, and at least one year of experience with developmental disabilities.

Qualifications

• Bachelor’s degree in a human services field required.
• Must meet qualifications to serve as a QIDP (QIDP eligibility or certification).
• At least 1 year of experience working with individuals with developmental disabilities.

Responsibilities

• Lead annual assessments and interdisciplinary team meetings for each resident’s Person-Centered Plan.
• Develop, update, distribute, and maintain Individual Service Plans and required resident documentation.
• Monitor resident progress, complete quarterly evaluations, and revise plans as needs or goals change.
• Coordinate with Home Managers and staff to ensure service plans are implemented correctly.
• Advocate for residents’ rights, preferences, goals, and community participation.
• Collaborate with residential, vocational, day program, therapy, family, guardian, and agency partners.
• Maintain resident records, confidentiality, required reports, and state-agency documentation.
• Assist with corrective action plans and compliance requirements from state agencies.
• Provide on-call crisis intervention and resident support as needed.
• Assist residents with benefit applications and required agency notifications.
• Participate in required training and perform other assigned duties.
