Job summary

Qualifications

• Experience in social services, preferably in a skilled nursing or long-term care setting.
• Bachelor's degree in Social Work, Psychology, Sociology, or related field preferred.
• Strong communication and interpersonal skills.
• Compassionate and resident-focused approach.
• Excellent organization and follow-through.
• Ability to work effectively with residents, families, and an interdisciplinary team.
• Knowledge of long-term care regulations and resident rights is a plus.

Responsibilities

• Advocate for residents and their rights, needs, and preferences.
• Develop meaningful relationships with residents and families.
• Assist residents with psychosocial needs, adjustment to the facility, and transition planning.
• Coordinate and participate in care plan meetings and family conferences.
• Maintain accurate and timely social services documentation.
• Assist with admissions, discharge planning, and transitions of care.
• Address resident and family concerns and help facilitate resolutions.
• Connect residents and families with community resources and services.
• Collaborate with nursing, therapy, activities, dietary, and other departments to provide person-centered care.
• Promote a welcoming, supportive environment for residents.
