Crest Health Care in the Greater Springfield Area, IL, is seeking a Social Services Director to lead our Social Services Department and champion residents’ emotional and social well-being.

You will collaborate with families and the interdisciplinary team to develop care plans, coordinate discharges, and advocate for residents’ rights and quality of life. Strong leadership and compassion are essential in this leadership role.

Qualifications

Your Role & Responsibilities

As our Social Services Director, you will help ensure residents' emotional, psychological, and social needs are supported while promoting their dignity, independence, and overall quality of life.

Responsibilities include:

• Oversee and manage the daily operations of the Social Services Department.
• Identify and address residents' psychosocial, emotional, and social needs.
• Complete social services assessments and develop individualized care plans.
• Participate in MDS assessments and documentation.
• Coordinate and facilitate resident care conferences with families and interdisciplinary team members.
• Build relationships with residents, families, and responsible parties.
• Coordinate resident discharges, including home health services, durable medical equipment, and transfers when needed.
• Advocate for residents while ensuring their rights, dignity, and well-being are respected.
• Connect residents and families with appropriate resources and support services.

What We're Looking For

• Previous social services experience in a skilled nursing or long-term care setting preferred.
• At least one year of social services experience preferred.
• Understanding of the MDS process and psychosocial assessments preferred.
• Strong communication, organization, and interpersonal skills.
• Ability to build positive relationships with residents, families, and staff.
• Compassionate, dependable, and team-oriented attitude.
