Job summary

Silver Birch Living is seeking an Eligibility Coordinator to help qualifying residents move into our community. You will gather documentation, work with residents and families, and coordinate with agencies to ensure all forms are completed for submission. The role supports our mission to create an inspired life for older adults.

Qualifications

• High School Diploma or equivalent required.
• 2+ years of experience with state services agencies (Medicaid, HUD, LITC).
• Strong customer service orientation.
• Ability to manage multiple tasks successfully.
• Bachelor's Degree in Social Work is preferred.
• Valid Driver's License with good driving record and automobile insurance.

Responsibilities

• Gather required documentation for qualifying residents to move into the community.
• Coordinate with resident and family to complete required documentation.
• Communicate with all necessary parties and coordinate assessment appointments.
• Prepare the resident’s administrative file for submission and approval.
