Job summary

Qualifications

Responsibilities

• Plan and implement recreational, social, and therapeutic activities for residents, tailored to their individual needs and preferences.
• Coordinate with the rehabilitation team to incorporate activities into individualized treatment plans.
• Maintain accurate records of resident participation and progress.
• Build relationships with residents, families, and staff to ensure a warm and welcoming environment.
• Participate in daily care conferences and contribute to care planning discussions.
• Provide support to other departments as needed, such as assisting with mealtime, transportation, and medication management.
