Job summary

MEMORIAL is seeking an Activity Assistant to support the Activity Director in delivering a creative, engaging activities program compliant with applicable standards. You will help design plans that meet the interests and needs of each resident and support a caring, respectful environment.

The role involves planning individual and group activities, coordinating calendars, arranging transportation, and assisting with documentation while ensuring safety and quality in a long-term care setting.

Qualifications

• High school diploma or equivalent.
• Preferable one-year experience in a long term care facility.
• Ability to read technical procedures.
• Ability to read and comprehend policy and procedure manuals.
• Ability to effectively present information and respond to questions from managers and employees.
• Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.
• Ability to solve practical problems.
• Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
• N/A (Certificates, Licenses, Registrations).

Responsibilities

• Plan and conduct individual, small and large group activities.
• Assist in providing good communication between employees at all levels, residents, families, support personnel, government agencies and the public.
• Participate in community planning related to facility interests and resident needs.
• Assist in development of monthly activity calendar and attendance records.
• Assist with action plans to correct quality deficiencies as requested by the Activities Director.
• Participate in discharge planning and development of activity care plans and assessments.
• Assist in arranging transportation for residents for outings, appointments, or discharge.
• Develop a monthly activity schedule including outings, group activities, and in-room activities for bed-bound or isolated residents.
• Encourage resident participation in hobbies, crafts, and reading; provide Braille or audio materials as needed.
• Assist with assessment documentation and quarterly progress notes.
• Keep the Activity Department clean, orderly and secure.
