Job summary

Las Colinas Post Acute is seeking an activities assistant to plan and conduct individual and group activities for residents. You will support communication among residents, families, staff, and external parties to meet residents' needs and promote well-being.

Responsibilities include developing monthly activity calendars, assisting with discharge planning and activity care plans, organizing transportation for outings, and encouraging participation in hobbies.

Qualifications

• High school diploma or equivalent.
• Preferable one year experience in a long term care facility.
• Ability to read and comprehend policy and procedure manuals; follow instructions accurately.

Responsibilities

• Plan and conduct individual and group activities for residents.
• Assist in communication between employees, residents, families, and the public to address needs.
• Develop and maintain monthly activity calendar and attendance records.
• Assist in discharge planning and activity care plan development.
• Arrange transportation for residents as needed for outings or appointments.
• Encourage participation in self-initiated activities and hobbies.
• Keep the Activity Department clean, orderly and secure.
