Job summary

Pruitthealth Corporation in Durham, NC seeks an Activities Assistant HCC to plan, implement, and evaluate recreational, social, intellectual, and emotional activities for residents as part of the care plan.

You will conduct assessments, adapt programs to abilities, work with the interdisciplinary team, and help ensure access and participation, including scheduling and documenting progress.

Qualifications

• Requires minimum two (2) years college education with courses in recreation, education, social work, other behavioral sciences or clinical services.
• Two (2) years' experience in a social or recreational program within the past five years, with one year full-time employment in a patient/resident activities program in a health care setting.

Responsibilities

• Assists in the planning, implementation, and evaluation of recreational, social, intellectual, emotional and spiritual programs per patient care plan.
• Conducts assessments by interviewing residents and families to develop individualized recreation programs.
• Updates programs to accommodate needs and protect residents' rights.
• Works with the interdisciplinary team to develop and evaluate the plan of care and communicate outcomes.
• Schedules activities across times and locations to engage all residents and posts monthly calendars.
• Provides in-room activities reflecting residents' life-long interests and supports participation.
• Delivers structured activities for cognitively impaired residents and adapts tasks for participation.
• Develops procedures to inform residents of activities and ensure transportation.
• Records progress notes in the clinical record and audits chart entries for accuracy.
• Performs quality assessment and assurance, rounds, and identifies staffing issues for corrective action.
• Develops and updates Recreation Services policies and procedures reflecting state and federal laws.

Education

Two years college education with courses in recreation, education, social work, other behavioral sciences or clinical services.
