Hillcrest Commons Nursing & Rehabilitation Center in Massachusetts seeks an Activities Assistant to plan, coordinate, and lead engaging resident activities. The role supports post-acute care teams across a broad range of programs, ensuring meaningful participation and well-being for residents.

Responsibilities

• Assist in planning, developing, organizing, implementing, and evaluating activity programs.
• Observe resident attendance, behavior, and level of involvement in activities.
• Encourage resident participation as deemed appropriate.
• Transport residents to and from activities and assist with discharge planning within activity care plans.
• Interview residents or family members to obtain activity information.
• Arrange transportation for outings and coordinate activities with other departments.
• Contribute to ongoing quality assurance for the activity department.
• Perform administrative duties such as completing forms and reports.
