Job summary

Talentify seeks an Activity Program Assistant in Wisconsin to support daily activity calendar requirements, align with the Activity Director's mission, and ensure high quality of life for residents. You will coordinate with staff, families, and the community to deliver person-centered activities and maintain records.

Qualifications

• High school education or equivalent.
• At least 1 year experience in healthcare, volunteering, or personal experience with disabled population.
• Knowledge of Federal/State activity regulations or ability to interpret.
• Able to read and write English.
• Valid WI driver's license.
• Punctual and time efficient with the activity calendar.
• Empathy, compassion, and positive disposition for residents, families, and staff.
• Able to relate to ill, disabled, elderly, and emotionally upset individuals.
• Able to communicate with community, health professionals, and organizations.
• Manage emotional needs of self and staff.

Responsibilities

• Provide feedback to Activity Director for monitoring facility and program goals.
• Lead a therapeutic, person-centered activity program to enhance quality of life.
• Follow Activity Policies, Procedures, and Federal/State regulations.
• Understand each resident's background to create person-centered activity plans.
• Ensure activities reflect residents' schedules, choices, and rights.
• Accommodate language services and communication needs of residents.
• Conduct and complete initial and ongoing assessments across domains.
• Document required information per Activity Director.
• Maintain records and assist in scheduling resident activities.
• Update activity boards monthly with creative themes.
• Facilitate community involvement and outings with residents.
• Work with staff and volunteers; adhere to schedules and continuity of care.
• Maintain activity schedule, avoid cancellations without approval.
• Maintain equipment and supplies; notify when replacements are needed.
• Update calendars and boards daily.
• Perform other duties as assigned.
