Job summary

Qualifications

Responsibilities

• Plan and lead individual, small-group, and large-group activities residents enjoy
• Build communication with residents, families, coworkers and outside partners
• Contribute ideas to the monthly activity calendar and track attendance
• Support quality improvement efforts with the Activities Director
• Participate in discharge planning and create personalized activity care plans
• Coordinate transportation for outings, appointments, and discharges
• Provide materials like Braille books or audiobooks as needed
• Assist with assessment documentation and quarterly progress notes
