• Safely transport residents to on-site and community-based activities and events.
• Track participation and help ensure residents have access to enriching experiences.
• Build relationships with local organizations and community partners to create engaging opportunities for residents.
• Ensure activity supplies and materials are available to support successful programs and events.
• Collaborate with the care team by providing input on resident goals, care plans, and overall well-being.
