Job summary

Intermountain Health is seeking an RN Hospital Care Manager I to coordinate comprehensive care for designated patients across inpatient and outpatient settings. You will develop patient-centered care plans and collaborate with families, providers, payers, and community partners to ensure seamless, high-quality care.

You will assess needs, guide self-management, and lead care transitions with strong clinical judgment and communication.

Qualifications

• Current Registered Nurse (RN) license in state of practice.
• BSN from an accredited institution with requirement to obtain BSN within 4 years if hired or promoted.
• Clinical nursing experience in an inpatient hospital setting.

Responsibilities

• Assess patients using screening criteria and conduct comprehensive evaluations.
• Develop, adjust, and coordinate care plans addressing medical and social needs.
• Guide self-management using motivational interviewing and coaching.
• Provide education to healthcare teams and patients; advocate for patient rights.
• Collaborate with multidisciplinary teams to ensure timely, high-quality care.
• Lead initiatives to improve care transitions and policy adherence.
• Provide Goals of Care education for informed decision-making.
• Track key metrics, analyze trends, and ensure regulatory compliance.
