Qualifications

• Current RN license in state of practice.
• BSN from an accredited institution; BSN must be obtained within four years if hired/promoted.
• Case management certification is preferred for the role.

Responsibilities

• Identify patients using screening criteria and conduct comprehensive evaluations.
• Develop, adjust, and coordinate care plans to address medical and social needs.
• Guide self-management using motivational interviewing and coaching.
• Provide education to healthcare teams and patients, advocate for patient rights, and support self-care.
• Collaborate with multidisciplinary teams to ensure timely, high-quality care.
• Lead initiatives to enhance care transitions and policy adherence.
• Provide Goals of Care education for informed decision-making.
• Track key metrics, analyze trends, and ensure regulatory compliance.
