Job summary

Intermountain Health is hiring an RN Hospital Care Manager I to deliver comprehensive care management across inpatient and outpatient settings. You will screen, assess, and develop patient-centered care plans with shared goals and tailored interventions.

The role collaborates with patients, families, providers, payers, and community partners to ensure efficient, high-quality care while leading process improvements and supporting advanced care planning.

Qualifications

• Current RN license in state of practice.
• BSN from an accredited institution; BSN must be obtained within four years of hire/promotion.
• Demonstrated clinical nursing experience in an inpatient hospital setting; familiarity with hospital terminology and processes.
• Understanding of disease management, treatment, LOS, barriers to delivery of care.

Responsibilities

• Assess patients using screening criteria and conduct comprehensive evaluations.
• Develop, adjust, and coordinate care plans addressing medical and social needs.
• Guide self-management using motivational interviewing and coaching.
• Educate care teams and patients; advocate for patient rights and self-care.
