Job summary

Harbor is seeking a Care Coordinator to support youth with complex needs and their families. The role is full-time, remote, and requires evening hours and weekend availability to engage families and attend community events.

Qualifications include 3 years of relevant experience (or 2 years with an Associate or Bachelor degree; or 1 year with a Master degree) in community-based services, with competencies in family systems and case management.

Qualifications

• 3+ years in community-based services for youth and families.
• Experience in child behavioral health, welfare, or related public services.
• Ability to manage complex cases and coordinate across systems.
• Clear verbal and written communication with diverse audiences.
• Proficiency with computers and documentation requirements.

Responsibilities

• Engage youth and families for assigned caseload (1:10 to 1:25).
• Link families to community resources and supports.
• Develop and monitor a strength-based care plan.
• Lead Family Team meetings and ensure family voice in plans.
• Coordinate clinical and psychiatric consultations when needed.
• Ensure CME documentation and engagement are maintained.
