Job summary

Harbor, Inc. is seeking a Care Coordinator to deliver comprehensive, multi-system care coordination for youth with complex needs and their families. The role is full-time, remote, with evening and some weekend hours, serving communities in Ohio.

Strong assessment, planning, and linkage to community resources are essential. Qualifications include 3+ years in related settings (or 2 years with an advanced degree) and CPR/First Aid, CPI/NCI within 90 days.

Qualifications

• 3+ years in providing community-based services to children, youth and families, or related public sector health services.
• Experience in family systems, community resources, case management, or child development.
• Culturally competent with experience managing complex cases.
• Experience with families experiencing behavioral health challenges and involvement with service systems.
• Complete high-fidelity wraparound training (COE) within 90 days.
• Clear verbal and written communication with diverse audiences.
• Valid Driver’s license and insurable vehicle.
• CPR/First Aid and CPI/NCI certifications within 90 days.
• Preference for staff residing in the service community.
• Reliable home internet with adequate speed.

Responsibilities

• Engages children/youth with significant behavioral health needs and their families for assigned caseload (1:10 ICC; 1:25 MCC).
• Maintains knowledge of community resources and links families to needed supports.
• Conducts comprehensive CANS assessment and develops a strength-based Care Plan.
• Monitors and updates the plan to address barriers and ensure service delivery.
• Leads Child and Family Team meetings giving the family a voice in planning.
• Facilitates access to clinical and psychiatric consultation for youth.
• Develops safety-crisis plans with youth and family.
• Ensures engagement, contacts and documentation meet CME rules.
• Willing to work flexible hours including evenings/weekends.
• Provides services in-home and in community settings.
