Job Responsibilities
- Develop and implement individual crisis and safety plans in conjunction with youth and family.
- Facilitate referrals and linkages, monitor care plan implementation.
- Manage 1915(c) Waiver funds ensuring prior authorization and ODM approval.
- Ensure referrals and linkages with appropriate assessments, support, and services.
- Respect youth and family rights to define goals and choose providers; monitor for family‑friendly, culturally competent supports.
- Conduct member outreach, assessments, and meetings in person or virtually based on preference.
- Operate at intensive‑care coordination (ICC) ratio 1:10; moderate‑care coordination (MCC) ratio 1:20; caseloads may mix ICC and MCC.
- Integrate evidence‑based strategies into Child and Family Team (CFT) meetings; demonstrate behavioral health management skills.
- Implement/document patient response to individualized care plans; complete behavioral assessments.
- Participate in regular team planning meetings; engage newly referred youth and family in care coordination.
- Partner with family to develop a Child and Family‑Centered Care Plan (CFCCP); identify client family team.
- Facilitate CFT meetings at least monthly, more frequently as needed.
- Complete comprehensive assessments including Child and Adolescent Needs and Strength (CANS) assessments.
- Engage youth and family in forming their unique CFT and preparing them as partners.
- Convene CFT meetings, ensuring convenience for family; implement trauma‑informed care principles and culturally responsive approach.
- Coach and develop new staff; enhance quality in care planning and relationship management.
- Coordinate care with all partners, including physical health providers and school personnel.
- Communicate ongoing CFCCP implementation progress; obtain cooperation and approval.
- Develop community resources to meet identified needs.
- Demonstrate higher engagement with patients and family; increase CFT participation.
- Accurately document patient care plans; bill for care coordination activities.
- Maintain quality documents, attendance records, and approvals in youth EHR; ensure CFCCP signed by youth, caregivers, and care coordinator within two weeks of meeting.
- Provide CFCCP to Aetna for quality review; initiate authorization process per ODM timeframe.
- Develop SMART goals with measures for achievement.
- Act as consultant for enhancements to materials and EHR documentation standards.
Job Qualifications
- Must have reliable transportation; 60% of work in field serving Clermont, Brown, Adams, and possibly Scioto counties.
- Behavioral Health Care Coordination Specialist I: high school graduate or equivalent with 3+ years relevant experience OR bachelor’s degree with 1-year relevant experience.
- Behavioral Health Care Coordination Specialist II: bachelor’s degree with 2+ years relevant experience and training or master’s degree with 1 year experience in family systems, community systems & resources, case management, child & family counseling or therapy, child protection, or child development.
- May require clinical license, certification, or training.
- Preferred experience: wraparound, meeting facilitation, community‑based work.
Benefits
- Pay range: Specialist I $22.18/hr - $27.73/hr; Specialist II $23.90/hr - $29.88/hr.
- Primary location: Vernon Manor.
- Schedule: full‑time, day shift.
- Weekly hours: 40.
