Job summary

Crest Health Care is seeking a fully remote Hospital Admissions Liaison to support weekend admissions for our portfolio of long-term care and skilled nursing facilities. You will review referrals from hospitals in the St. Louis region, Springfield area, and surrounding communities.

This role offers three 12-hour shifts on Friday, Saturday & Sunday, 36 hours per week, and a pay rate of $23–$25 per hour. Equal opportunity employer and remote-first position.

Qualifications

• Long-term care or skilled nursing admissions experience required.
• Ability to read and interpret hospital referrals, medical records, and clinical documentation.
• Strong computer skills and ability to navigate multiple referral systems; organized and able to work independently.

Responsibilities

• Review and screen incoming hospital referrals
• Identify key clinical and admissions information
• Create a clear, concise synopsis of each referral
• Route referrals to the appropriate facilities and admissions teams
• Communicate with facility teams to help coordinate weekend admissions
