Job summary

BJC Home Care is seeking an Admissions Hospice Nurse to support hospice patients and families in the Greater St. Louis area. This full-time role offers a weekend schedule with a 36-hour week and mileage reimbursement; experience in admissions or hospice is preferred, and a RN license is required.

The role involves coordinating care across a multidisciplinary team, documenting plans of care, and ensuring compassionate, patient-centered support in patients' homes or facilities.

Qualifications

• Education: Nursing Diploma or Associate's degree (required).
• Preferred: Bachelor’s Degree in Nursing or Home Health.

Responsibilities

• Assess patient needs and plan care with the interprofessional team.
• Coordinate care as case manager and ensure adherence to nursing guidelines.
• Document plans of care and communicate updates to family and team.
• Ensure safety and quality outcomes through evidence-based nursing practice.
