Hallmark Healthcare of Carlinville is seeking a Hospital Admissions Liaison for a fully remote, weekend role covering Friday through Sunday. You will review referrals from hospitals across the St.

Louis region, Springfield area, and surrounding communities, coordinating admissions with long-term care and skilled nursing facilities. This full-time position offers three 12-hour shifts (36 hours weekly) and pays $23–$25 per hour.

Qualifications

• Long-term care or skilled nursing admissions experience required.
• Must be able to confidently read and interpret hospital referrals, medical records, and clinical documentation.
• Nursing or clinical background is recommended, but not required.
• Strong computer skills and ability to navigate multiple referral systems.
• Organized, detail-oriented, and able to work independently.
• Must reside in IL or MO.

Responsibilities

• Review and screen incoming hospital referrals
• Identify key clinical and admissions information
• Create a clear, concise synopsis of each referral
• Route referrals to the appropriate facilities and admissions teams
• Communicate with facility teams to help coordinate weekend admissions
