Job summary

Devoted Health seeks a Virtual Primary Care Nurse Practitioner or Physician Assistant to deliver high-quality primary care via telehealth. You will manage chronic conditions, perform preventive care, and coordinate with a multidisciplinary team to arrange labs, imaging, and referrals as needed.

You will work in Pacific Time hours, providing patient-centered care and continuously improving the virtual care experience.

Qualifications

• Active Nurse Practitioner (APRN) license in good standing with ability to obtain licensure in multiple states within 90 days.
• Board certification in Family Medicine, Adult-Gerontology, or Internal Medicine.
• Minimum 2 years of primary care, urgent care, or internal medicine experience, preferably with telehealth.
• Active BLS certification at time of hire.
• Able to work Pacific Time hours and adapt to variable shift times.

Responsibilities

• Provide virtual primary care visits via telehealth with comprehensive assessments and chronic disease management.
• Diagnose and manage conditions (hypertension, diabetes, COPD, heart disease) and develop care plans.
• Evaluate and treat acute concerns within virtual care scope; order and interpret labs/imaging.
• Manage medications, titrations, and refills with emphasis on safety and adherence.
• Provide health coaching and patient education to empower patients.
• Coordinate with specialists and community resources for referrals and in-person tests when needed.
• Contribute to the telehealth platform by giving feedback and participating in team reviews.
• Stay current with best practices in virtual and primary care management.
