Job summary
Vitae Health Systems in the United States is seeking a VBC Operations Program Manager to lead the LEAD ACO operating cadence and performance tracking. This hands-on role drives beneficiary alignment, utilization, quality, and compliance through clear actions and owner accountability.
Remote role with limited travel to facility partners and team meetings. You will run weekly huddles, monthly reviews, and quarterly improvement cycles with cross-functional teams, turning data into prioritized
Qualifications
- Bachelor’s degree in healthcare administration, public health, business, nursing, or related field.
- 4+ years in value-based care, ACO operations, or population health.
- Strong data analysis and ability to translate data into action.
- Excellent communication with clinicians and leadership.
- Experience with CMS alignment and quality measures is a plus.
Responsibilities
- Lead the LEAD ACO operating cadence and performance tracking.
- Own CMS reports and data feeds; flag variances.
- Maintain master CMS calendar with owners.
- Oversee CMS alignment lists reconciliation and root-cause reviews.
- Coordinate quarterly improvement cycles with cross-functional partners.
