Job summary

Vitae Health Systems in Chicago (remote with limited travel) is seeking a hands-on VBC Program Manager to drive the LEAD ACO operating cadence, track performance, and convert data into prioritized actions. You will own the calendar, cadence, and follow-through, while clinical standards remain with clinical leadership.

You will partner with the VBC clinical team, data resources, and operations managers to ensure timely, compliant performance reporting and continuous improvement across CMS

Qualifications

• Bachelor’s degree in healthcare administration, public health, business, nursing, or related field.
• 4+ years in value-based care, ACO operations, or population health.
• CMS ACO model experience (ACO REACH, High Needs, MSSP, or LEAD).
• Experience in post-acute, SNF, or long-term care.
• 2+ years managing staff.
• Track record running recurring operating cadences and deadlines.
• Experience with structured improvement cycles (PDSA, Lean).
• Working knowledge of CMS alignment, ACO quality measures, and related requirements.
• Ability to drive follow-through across clinical, operational, and data teams.
• Fluent in data reporting with Excel and BI dashboards.

Responsibilities

• Lead LEAD ACO cadence and performance tracking weekly and monthly.
• Monitor CMS reports, quality measures, and plan milestones.
• Ensure named owners for CMS deadlines and deliverables.
• Oversee CMS alignment reconciliation and attrition reviews.
• Coordinate cross-functional workflows with operations, clinical, and data teams.
• Prepare performance summaries for senior leadership and governance.
• Lead quarterly improvement cycles and document action plans with owners.
• Coach VBC operations team and develop SOPs; escalate as needed.
