Job summary

Included Health is seeking board-certified Family Nurse Practitioners licensed in New Hampshire and other states to provide virtual comprehensive care across all ages.

The role requires 3+ years in high-volume urgent or primary care, active RN and multi-state licensure (5+ licenses preferred), and telemedicine experience; remote-first with 32 hours/week full-time options.

Compensation includes base plus equity and benefits; weekend/evening shifts and holiday coverage are included in the schedule.

Qualifications

  • Graduate of an accredited Master’s or Doctoral-level Family Nurse Practitioner (FNP) program.
  • Active Nurse Practitioner (NP) license with maintained Registered Nurse (RN) licensure, in accordance with state requirements.
  • Current national certification as a Family Nurse Practitioner through AANP or ANCC.
  • Active RN compact license.
  • Active multi-state licensure (5+ licenses preferred).
  • Minimum 3+ years post-training experience in high-volume Urgent Care or Primary Care settings.
  • Experience providing care to newborns through adult populations.
  • Ability to function within an integrated medical practice.
  • Strong clinical acumen and ability to manage a broad range of conditions.
  • Excellent communication, collaboration, and patient engagement skills.
  • Proficiency with technology and electronic medical records.
  • Maintain active Medicare status – In good standing with Medicare/Medicaid and not excluded from participation in federal health programs.
  • Prior telemedicine or video-based virtual care experience.
  • Experience with Athena EMR.

Responsibilities

  • Conduct age-appropriate history and virtual exams.
  • Evaluate, diagnose, and treat a broad range of conditions across the lifespan.
  • Order and interpret diagnostic tests as needed.
  • Develop and document treatment plans in a timely, accurate manner.
  • Manage acute, chronic, and mental health conditions for pediatric and adult patients.
  • Request consultation or referral with other health care providers when appropriate.
  • Counsel and educate patients and families on treatment plans.
  • Maintain current knowledge of clinical best practices and evidence-based care.
  • Provide cross-coverage for tasks such as reviewing lab results.
  • Offer scheduling flexibility, including evenings and weekends.
