Spring Health is seeking an independently licensed mental health professional (LCSW/LMFT/LPC) to join the Care Navigation team. The Trauma Clinical Care Navigator will conduct initial assessment, support, and ongoing care coordination for PTSD and Complex Trauma, reporting to the Trauma team lead.

This is a full-time, fully remote role with typical business hours. Responsibilities include telephonic screening, stabilization, referral, and collaboration with multidisciplinary teams to ensure

Qualifikationen

• Licensed mental health professional (LCSW/LMFT/LPC) with PTSD experience.
• 2+ years of clinical care experience with PTSD and Complex Trauma.
• Experience with motivational interviewing, diagnostic assessment, and triage.

Aufgaben

• Telephonic screening, assessment, and referral for trauma care.
• Apply active listening, psychoeducation, and motivational interviewing to support goals.
• Use phased-based trauma treatment to provide stabilization.
• Coordinate care with Spring Health providers and identify level of care.
• Document records and communicate with members and providers via email.
• Collaborate with care support, provider operations, and benefits/billing teams.
• Participate in meetings, case consultations, and trainings.
• Maintain confidentiality and ethical standards.
