Job summary

Spring Health is seeking an independently licensed mental health professional to join the Care Navigation team, focusing on PTSD and complex trauma. The Trauma Clinical Care Navigator will assess, refer, and coordinate care for members seeking specialized trauma support.

This full-time, fully remote role requires licensure (LCSW, LMFT, LPC) and 2+ years of clinical PTSD experience. The position follows a Monday–Friday daytime schedule and collaborates with multidisciplinary teams.

Qualifications

• Experience with motivational interviewing, diagnostic assessment, and triage.
• Knowledge of phased-based trauma treatment model and EBPs for PTSD (e.g. Prolonged Exposure, CPT, EMDR, ART).
• Excellent listening, written and verbal communication skills.
• Tech-savvy and comfortable with multiple computer platforms.

Responsibilities

• Telephonic screening, brief intervention, and referral to trauma providers.
• Apply active listening and psychoeducation to support member goals.
• Provide stabilization and motivation to engage in care.
• Coordinate care with Spring Health providers and other teams.
• Document and maintain accurate records in the EHR within required timeframes.
• Follow confidentiality, compliance and ethical standards.
