Job summary

Molina Healthcare is seeking an experienced RN Case Manager for its Texas Health Plan. The role focuses on high-need pregnant members, performing telephonic assessments, care coordination, and service management from home.

The ideal candidate has 2+ years in health care, care management, and experience with LTSS or disabilities. Travel 25–40% may be required; strong MS Office proficiency is essential.

Qualifications

• Registered Nurse (RN) with active license in state of practice.
• Experience in care management or managed care settings preferred.
• Experience with waiver services and LTSS is a plus.

Responsibilities

• Conduct comprehensive member assessments within timelines, including in-home visits as required.
• Develop and monitor care plans and waiver service plans in collaboration with members and providers.
• Coordinate services across behavioral health and LTSS.
• Monitor for medical necessity and authorize waiver services as appropriate.
• Facilitate interdisciplinary care team meetings and document interventions.
