Molina Healthcare is seeking a licensed RN Case Manager in Texas to support women's health and LTSS coordination from a home-based setting. You will evaluate member needs and lead care coordination for high-risk pregnancies, using the Molina clinical guideposts and motivational interviewing.

Typical duties include comprehensive assessments, waiver enrollment support, care plan development, and multidisciplinary collaboration. Travel 25–40% may be required based on state requirements.

Qualifications

• 2+ years in health care with care management or managed care experience.
• RN license active and unrestricted in state of practice.
• Experience with LTSS, disabilities or chronic conditions preferred.
• Proficiency in Microsoft Office (Outlook/Excel/Teams).

Responsabilités

• Complete comprehensive member assessments within timelines, including home visits as needed.
• Develop and implement waiver/service plans with members and care teams.
• Monitor care plans, document interventions, and adjust as needed.
• Coordinate services across behavioral health, LTSS, and community resources.
• Assess medical necessity, authorize waiver services, and manage benefits.
