Responsibilities

• Telephonic engagement with members to coordinate care.
• Perform member outreach and engagement, including hard-to-reach members.
• Develop individualized care plans with goals and interventions.
• Maintain accurate documentation in the member's electronic health record.
• Collaborate with interdisciplinary care team to address barriers to care.
• Connect members with health plan benefits and community resources.
• Ensure regulatory compliance within timelines.
• Consult with Care Manager RN for clinical knowledge and decisions.
