Job summary

Humana Inc. seeks a Care Manager, Telephonic Behavioral Health 2 to assess members' needs in a telephonic setting and guide interactions with resources to enhance wellness. The role requires a social service license and a Master’s in behavioral health, plus 2+ years of relevant experience.

You’ll use standard tools (Word, Excel, PowerPoint) to document care plans, monitor progress, and coordinate with care teams, with occasional travel for training as needed.

Qualifications

• Current valid social service license (LMSW/LCSW/LPC/LMHC)
• Master's degree in behavioral-health field from an accredited university
• 2+ years or more of experience as a behavioral health professional

Responsibilities

• Assess and evaluate members' needs to achieve optimal wellness state
• Guide members toward resources and facilitate interaction with care providers
• Monitor patient progress through assessments/evaluations and adjust care plans as needed
• Understand department and organizational strategy and follow guidelines/procedures
