Job summary

Fuze Health is seeking a Registered Nurse to support the telehealth care team, providing remote outreach to Medicare members and ensuring adherence to Stars measures. Responsibilities include virtual assessments, documenting patient histories, and educating patients on care plans.

Schedule: Monday–Friday 12:30 PM–9:00 PM ET with one Saturday monthly. Requirements include a BSN, an active compact state license, 2+ years RN experience, and 1–2 years in telehealth.

Qualifications

• BSN degree and active, non-restricted nursing license.
• 2+ years of RN experience.
• 1-2 years in a remote healthcare or telehealth setting.
• Strong computer skills and familiarity with EMR systems.
• Clear, effective written and verbal communication.
• Ability to follow SOPs and adapt in fast-paced environments.
• Customer service and interpersonal skills in high-volume situations.

Responsibilities

• Provide remote telehealth patient care and support.
• Conduct virtual assessments reviewing medical histories and symptoms.
• Accurately document patient conditions and care interactions.
• Educate patients and families on conditions, medications, and self-care.
• Collaborate with other providers to develop individualized care plans.
• Maintain regulatory compliance and privacy standards.
• Pursue continuing education and stay updated on telehealth practices.
• Demonstrate compassionate, professional communication with patients.
